Money · Check the payer

Ask about the use as well as the drug.

Ask the plan for its own answer

A drug can be eligible for coverage and still be subject to the plan’s medicine list and prior authorization.

Prior authorization is permission your plan must give before you fill a prescription using that coverage. Ask what records it needs and who submits them.

  • Is this exact product covered for my diagnosed condition?
  • Does a visit charge apply even if the drug is denied?
  • What would I pay at the pharmacy?
  • How long does an approval last, and what is needed to renew it?

Medicare has separate routes

These US insurance and Medicare payment rules were checked on September 26, 2026.

Medicines for weight loss fall outside the usual Medicare drug benefit.

Mounjaro (tirzepatide) for type 2 diabetes and Zepbound (tirzepatide) for sleep apnea can instead be eligible for ordinary drug-plan coverage.

Eligible patients can obtain selected weight-loss drugs for $50 a month through the Medicare GLP-1 Bridge, a separate program that is temporary.

July 1, 2026 is the Bridge’s opening date; the closing date is December 31, 2027.

For Zepbound, the Bridge covers only the KwikPen, a pen holding several doses. It excludes single-dose pens and vials, which are small medicine bottles.

Check the Bridge rules against your records

Patients must be at least 18; an age of 80 does not on its own prevent eligibility.

A prescription and advance approval from the plan are required.

Eligibility also depends on weight compared with height, called body mass index or BMI. Ask the prescriber to check the following conditions against your health records at the time treatment began.

The health rules allow a BMI of 35 or more without an added health condition.

At BMI 30 or higher, either the specified type of heart failure or kidney disease at stage 3a or beyond can support eligibility. It can also qualify with blood pressure still above the program’s limits despite two blood-pressure medicines. The program’s blood-pressure limits are above 140 for the top reading or above 90 for the bottom reading.

Ask the prescriber to check the exact heart diagnosis, kidney stage, or blood-pressure readings against the full terms linked in the source file.

For BMI 27 or higher, the terms list pre-diabetes, an earlier heart attack or stroke, and the specified artery disease among the qualifying conditions.

With pre-diabetes, blood sugar has risen beyond normal levels without reaching the range for diabetes. Ask the prescriber to confirm the exact diagnosis; a general description of heart or artery trouble is not enough.

You need either a separate Medicare Part D drug plan or an eligible Medicare Advantage plan that includes drug coverage.

Medicare Part D payment for a GLP-1 drug during 2026 prevents that person from using the Bridge during 2026.

The terms also require ongoing changes to eating and physical activity.

The ordinary drug plan handles coverage for type 2 diabetes, and for moderate to severe obstructive sleep apnea. During sleep apnea, a person’s breathing keeps stopping or becoming too shallow while asleep.

The terms also send certain people with liver disease, or a prescription for heart and stroke prevention, to ordinary drug-plan coverage instead.

Payments under the Bridge add nothing toward the usual annual drug-spending cap, and using coupons alongside them is not allowed.

These are checks to discuss, not a promise of payment. The source file links to the full terms, including the exact health conditions and excluded plan types. Have both the prescriber and your plan identify the payment route for your case.

Do not confuse a savings card with coverage

Maker savings cards require workplace or private insurance and exclude Medicare, Medicaid, and military health coverage.

A maker’s cash offer may still be available to someone on Medicare who pays without insurance.

For tirzepatide, ask about the maker’s Zepbound cash offer described on the full-price page, and confirm whether its terms allow you to use it.

Ask which offer is being discussed, which medicine and container it covers, and what its written terms require.
